FATAL FRAME: Maiden of Black Water is een verbeterde versie van de gelijknamige Wii U-release uit 2014, waarmee spelers kunnen genieten van deze unieke horrorervaring. Spelers kruipen in de huid van Yuri Kozukata, Ren Hojo en Miu Hinasaki terwijl ze de griezelige berg Hikami opgaan in de hoop degenen te vinden die eerder zijn verdwenen. Om de kwaadaardige geesten die zich in de wateren van de bergen bevinden te verslaan, moeten ze de Camera Obscura onder de knie krijgen - een unieke camera die geesten kan schaden wanneer ze een foto maken. Elk succesvol schot zorgt ervoor dat een geest Spirit Fragments uitzendt en als spelers een afbeelding van de geest en deze mysterieuze fragmenten in hetzelfde frame kunnen vastleggen, ontketenen ze een Fatal Frame-aanval met hoge schade. Door de onderdelen van de camera te upgraden, zoals het uitrusten van een sterkere lens of het vervangen van de film, kunnen krachtigere foto's worden gemaakt, waardoor spelers meer bescherming krijgen als ze verder de berg op gaan. Wanneer een geest is verslagen, kan de camera de Glancing-vaardigheid uitvoeren om het verleden van de geest bloot te leggen en zo de waarheid achter de verdwijningen op de berg te achterhalen.

Nieuw in deze verbeterde versie is een gloednieuwe fotomodus waarin spelers elk personage of geest uit hun avontuur kunnen gebruiken, ze op een achtergrond naar keuze kunnen plaatsen en een verscheidenheid aan effecten kunnen toevoegen (lenstype, framekeuze, focus, enz. ) om de perfecte enge foto te maken. Er zijn ook nieuwe kostuums en accessoires toegevoegd, waaronder Yuri's 'Punk Ensemble', 'Cutesy Goth Ensemble (Black)', 'Swimsuit (Blue)' en het 'Triathlon Suit', evenals Miu's 'Miku Hinasaki Outfit (The Tormented)' , 'Cutesy Goth Ensemble (Wit)', 'Swimsuit (Yellow)' en 'Swimsuit (Pink)', samen met Ren's 'Groom's Outfit'.
