De verhoogde detectie van methylmalonzuur in de urine vormt een indirecte marker van een vitamine B12-defici�ntie. Het maakt het mogelijk om conclusies te trekken over het metabool beschikbare vitamine B12. Vitamine B12 fungeert als een co-enzym van methylmalonyl-CoA mutase die in de citroenzuurcyclus methylmalonyl-CoA katalyseert tot succinyl-CoA. Wanneer deze stofwisselingsroute bijv. ten gevolge van een verminderd aanbod van vitamine B12 verstoord is treed een verhoogde excretie van methylmalonzuur op.

De bepaling van methylmalonzuur in de urine is te verkiezen boven de bepaling in het serum: het is eenvoudiger en gevoeliger. De diagnostische waarde van de parameters in serum wordt vaak beperkt doordat vitaminen in het serum gebonden zijn aan bindingseiwitten waardoor de vitamine B12-verzorging op cellulair niveau niet altijd voldoende kan worden bepaald.

Veelvoorkomende oorzaken van een vitamine B12-tekort zijn gastro-intestinale aandoeningen chronische atrofische gastritis en misbruik van alcohol en/of� medicijnen. De gevolgen van het gebrek uiten zich met name in stoornissen in de erytropo�se (megaloblastaire anemie) glossitis gastritis obstipatie en in neurodegeneratieve en psychiatrische symptomen (neuropathie dementie geheugenstoornissen en psychosen).

De uitslagen dienen als basis voor een individuele therapie en moeten met een arts of therapeut worden besproken.
